Fee for full 3-day workshop plus 4th day Forum:
$2,000 (CAD) per person for Public/academic sector.
$2,500 (CAD) per person for Commercial sector.
Fees for full 3-day workshop plus 4th day Forum are fully inclusive of tuition and course materials but do not include accommodation. Breakfasts & lunches are provided during the course as well as a cocktail hour and dinner.
Fee for 4th day Forum only:
$400 (CAD) per person for Public/academic sector.
$500 (CAD) per person for Commercial sector.
Fees for 4th day Forum only are fully inclusive of tuition, material presented but do not include accommodation. Breakfast & lunch will be provided.
Cancellation:
The fee is payable in advance. A refund of course fees (less 10% administrative charge) will be allowed for cancellations received in writing prior to September 18, 2010. Cancellations made after September 18, 2010 are not refundable. Substitutions are allowed but please fax new delegate details when known.
Please note: In the unlikely event that, due to unforeseen circumstances, the course has to be cancelled by the PATH Research Institute, St. Joseph’s Healthcare, our liability is limited to refund of the workshop fees only. We recommend delegates have adequate insurance coverage to claim any travel or personal expenses.

Credit Card payments are available (VISA, AMEX and MasterCard only), please fill in the credit authorization form and either email the form to hendersc@mcmaster.ca or fax to (905) 522-0568 (If using the credit card form please type the info onto the form). Once we receive your registration we will contact you with the payment options at that time. We will also forward a copy of the credit card authorization form to you at that time. A receipt will be sent to you after the payment transaction has been approved/processed.
