2012 Fees for 3-day workshop plus 4th day Forum:

Registration(Public/Academic sector): $2,500 (CAD)

Registration (Commercial sector): $3,000 (CAD)

Fees are fully inclusive of tuition, breakfasts, lunches, gala dinner and course materials (accommodation is not included).

2011 Fees for 4th day Forum only:

Public/Academic sector: $400 (CAD)

Commercial sector: $500 (CAD)

Fees are fully inclusive of tuition, breakfast, lunch and course materials (accommodation is not included).

Cancellation

Fees will be payable in full less 10% cancellation fee, when cancellation is received in writing by September 18, 2012. After September 18, there will be NO refund and participants will not be allowed to transfer registration to following year(s).

Please note: In the unlikely event that, due to unforeseen circumstances, the course has to be cancelled by the PATH Research Institute, St. Joseph’s Healthcare, our liability is limited to refund of the workshop fees. We recommend delegates have adequate insurance cover to claim any travel or personal expenses.
