For registration at the workshop taking place in: 

Hong Kong, please use the registration form below.

  • Non-commercial rate:  $1250 Canadian dollars
  • Commercial rate:  $1900 Canadian dollars

Attendees will be provided with a copy of materials presented at the workshop and the fees above include lunches and refreshments during breaks.  Please note that accommodation is not included in the registration fee.  All attendees will be responsible for booking and paying for their accommodation while at the workshop.

We accept AMEX, MasterCard or VISA.  If paying by cheque, please make cheque payable to St. Joseph's Healthcare Hamilton and mail to the attention of Christine Henderson, PATH, 25 Main St. West, Suite 2000, Hamilton, ON. L8P 1H1 Canada.

This workshop will be held in Room 205S, 2/F, Hospital Authority Building, 147B Argyle Street, Kowloon, Hong Kong.

Cancellation Policy:

A full refund of course fees (less 10% administrative charge) will be made for cancellations received in writing at least one month prior to the workshop. Substitutes can be made but please email new delegates details when known to hendersc@mcmaster.ca. Cancellations made less than one month prior to the workshops are non-refundable/non-changeable.

In the unlikely event that, due to unforeseen circumstances our liability is limited to refund of workshop fees. We recommend delegates have adequate insurance cover to claim any travel or personal expenses.
